New Patient Forms
Here’s some common forms we’ll need filled out during a New Patient appointment. You can download/print the forms from the links below:
- Appearance release form (to allow us to take in-office photos/videos and use them in our marketing, i.e. brochures, social media, or website)
- Financial and Appointment policy (please fill out form, even if you don’t have insurance)
- Insurance and Responsible Party Information (for new patients or if you get new insurance)
- New Patient Demographics and Health History form
- HIPPA Privacy form
Getting your child ready for their appointment
Our goal is to make every appointment as fun and easy as we can! We love hearing our patients tell their parents that they’re excited to come back and see us. As a parent, you are a vital part of the team to help make their appointment successful. Before the appointment, please discuss the positive aspects of dentistry with your child. One way to convey good feelings to your child about dental visits is to remind them that going to the dentist is a sign that they are “growing up”. You can explain that the doctors will count their teeth, as well as take pictures of his/her teeth. Please do not tell your child that the “dentist will not hurt” as this may never have entered his/her mind. Instead, you may wish to assure your child that the doctors will be gentle and friendly. Also, please avoid using words needle, shot, pull, or any other words suggesting unpleasantness. Expect your child to do well and enjoy their visit to our office and chances are he/she will do exactly that!
To limit any anxiety that your child might have about the appointment, we want to use child-friendly terms to explain the instruments and procedures that may be involved. Here is a list of terms that we have found will allow you to explain what will happen at your child’s appointment without overwhelming them.
Normal Dental Term vs Child-Friendly Term
At your appointment, our staff will review the health history form with you. You and your child will be introduced to our dental team and invited to tour the office. Dr. Rubin or Dr. Sentelle will perform a head and neck examination and then evaluate the health of the teeth, gums and your child’s bite. Dental radiographs are taken only as the doctors feel would be necessary. Additional services will be scheduled for a later visit.
